New U.S. Passport for  adults

Use this procedure if:

  • You are 18 and older (if you are between 16-18 years of age, please follow Passports for 16-18yr Olds Procedure)
  • You are over 18 years of age and had a passport valid for 5 years issued to you as a minor
  • Have never had a U.S. Passport before
  • Your U.S. Passport was issued more than 15 years ago

STEP 1. Gather the following documents:

  • Proof of U. S. Citizenship (certified birth certificate with both parent's names; previous original US passport, or original Certificate of Naturalization) plus 1 copy
  • Copy of valid I.D. (Driver's license or state issued ID)
  • Two passport size photos 2"X 2"  (facing the camera with white background, must be taken within last 2-3 months), cannot wear glasses (new Passport Agency rule as of 11/01/2016)
  • One Application DS-11 (fill out online or download paper version). This form can only be signed in front of the clerk at the post office
  • Two Letters of authorization - (click here to download) once you sign it, we have to submit it to the passport agency within 5 business days. Signature(s) on the letters of authorization and all other documents must match signatures on your ID.

STEP 2. Document Pre-Check:

Passport Plus Visas Document Pre-Check Service can save valuable time and reduce stress. Use this service to avoid mistakes or delays. Our experienced passport specialists will review and check your scanned documents to confirm that your application is ready to be filed, and will alert you if there is information that needs to be corrected or if there any missing paperwork. E-mail copies of the documents from Step 1 to for confirmation.

STEP 3. Find the nearest post office that can assist you by clicking here.

  • Show clerk your drivers license or valid passport and tell him/her that you want to do a "hand-carry" procedure
  • Sign the application form DS-11 in front of the clerk, so they can witness your signature
  • Make a money order or check for $170.00 made out to "US Department of State"
  • Present all documents from the list in STEP 1 to the post office clerk
  • Post office clerk will put all documents into one envelope, seal it, put official post office stamps on it, and give it back to you
  • Make sure to ask the post office clerk to address the envelope to "Passport Agency" and put your name and date of birth on it
  • The post office will charge you $35.00 for this process
  • Make sure that ALL paperwork including itinerary is included inside sealed envelope. Place one Letter of Authorization inside and one on the outside of the sealed envelope.
  • WE MUST RECEIVE YOUR SEALED ENVELOPE WITHIN 24 HOURS AFTER IT'S SEALED. If it takes longer for the sealed envelope to reach our office, the Passport Agency might ask to re-seal the envelope.

STEP 4: Fill out Order Form (Download here) and place it on top of your sealed envelope.

STEP 5: Send or deliver all documents for processing to the address below:

*Please include our service fee and return delivery payment or a completed air bill. Incomplete payment, incomplete applications or documents and wrong application form will cause a delay in processing your application.
** The service fees are non-refundable, once we receive your documents and start processing them. Due to the high volume of applications being submitted, the processing times may be delayed by the Passport Agency. While such situations are beyond our control, we will do everything possible to accommodate your trip and will call you immediately to let you know about the status of your application, the service fees will not be refunded. If the Passport Agency requires additional paperwork or additional originals of the Letters of Authorization, photos, applications or any other documents from the applicant, the date of completion will change to a later date provided by the Passport agency. PPV does not charge for additional trips to the Agency. The applicant is responsible to provide additional paperwork and cover the expense of getting it to our office.
