NEW U.S. Passport for minors (under 16 years of age)



Use this procedure if:

  • Your child is under 16 years of age 
  • If your child has never had passport before


STEP 1. EMAIL COPIES OF THE DOCUMENTS TO INITIATE YOUR RESERVATION TO INFO@PASSPORTPLUSVISAS.COM:

  • Copy of US birth certificate with both parents' names on it, must be issued by the state, NOT the hospital
  • Copies of both parents' driver's licenses (front & back)
  • Copy of itinerary showing international flights (must include the child's & at least one parent's name)
  • One Application DS-11 (click here to download)
  • PPV Order Form (click here to download)
  • Application DS-3053 (click here to download)- this form is only needed IF one of the parents cannot go to the post office. The non-appearing parent must fill out and notarize this form.


STEP 2: RESERVATION CONFIRMATION

  • Your paperwork will be reviewed and your will be contacted by one of our agents.
  • We will provide all required paperwork for your passport renewal BY EMAIL along with SPECIFIC INSTRUCTIONS on how to print, sign and where to mail your originals. DO NOT MAIL ORIGINALS UNTIL YOU RECEIVE YOUR RESERVATION CONFIRMATION.
  • We will provide a confirmation of your reservation and a specific date when your documents can be submitted for processing. Once the date is reserved, your credit card will be charged for the total amount and the service fee becomes non-refundable. Receipt will be provided for your records.
  • You must follow instructions provided by our agent. Failure to follow instructions can result in cancellation of your reservation. Service fee is non- refundable.
  • Please allow 24 hours for review and confirmation.



STEP 3: VISIT YOUR LOCAL POST OFFICE

  • Both parents AND the child must appear at the post office. If one parent cannot appear, they must provide a notarized form DS-3053.
  • Before you can mail your originals to us, a special procedure needs to be done at your local post office. It's called HAND-CARRY PROCEDURE.
  • Appointments are required at most post offices due to Covid restrictions. We will help you make an appointment and confirm your availability.
  • A special post office clerk will need to witness your signature and seal all of your documents in one envelope.
  • Once your paperwork is sealed, it will be valid for maximum of 5 business days. We'll need to receive your sealed envelope and submit it to the passport agency within those 5 days. Because it's a time-sensitive procedure, a lot of planning & coordination will be required. Our agents will be communicating with you throughout the whole process, so we can all stay on the same page and you'll know exactly what to expect. 
  • Missing your post office appointment or doing hand carry procedure too early or later can result in cancellation of your reservation. Service fee is non-refundable.



STEP 4: SEND YOUR SEALED ENVELOPE TO THE ADDRESS PROVIDED IN YOUR RESERVATION CONFIRMATION

  • We must receive your sealed envelope during the time specified in your reservation confirmation.

  • Reservation for each submission must be made & confirmed by our agents PRIOR to mailing your originals to us. We will NOT honor requests mailed to our office without prior reservation
  • Do not call our office to make a reservation on the phone, it can only be done by following directions below
  • All reservations will be first-come first-serve
  • Once your reservation is confirmed, the service fee is non-refundable. No exceptions.
  • We will not be able to accommodate all requests due to the limited number of applications that can be submitted daily. As processing capacity expands, we will update our website with additional options.

*Please note that Passport Plus will not be able to get involved after your original documents are mailed to the National Passport Processing Center or contact the Passport Processing Center on your behalf. If additional documents or information is requested by the government and you need assistance in understanding the communications, putting together additional paperwork and submitting, additional fees will apply.
