Use this procedure if:

  • Your child is under 16 years of age 
  • If your child has never had passport before


STEP 1. Gather the following documents:

  • Copy of minor's birth certificate
  • One passport size photo 2"X 2"  (facing the camera with white background, must be taken within last 2-3 months)
  • One Application DS-11 (click here to download)
  • Copy of parents' ID's (front & back)
  • PPV Order Form (click here to download)


STEP 2. E-mail copies of all documents to info@passportplusvisas.com

  • E-mail copies of all documents listed in Step 1
  • Wait for final applications and instructions

*Please note that Passport Plus will not be able to get involved after your original documents are mailed to the National Passport Processing Center or contact the Passport Processing Center on your behalf. If additional documents or information is requested by the government and you need assistance in understanding the communications, putting together additional paperwork and submitting, additional fees will apply.
